Community Care Alliance (CCA) provides a suite of services for our participants. We offer training, resources, materials and education to the right person on your team so you can provide the care your patients deserve while achieving value-based care.

Our rural health participants are leaders in quality and patient outcomes.

Along with standard services, conferences and email updates, we provide practical services with proven results unique to the rural areas who geographically work in silos. Through role-specific forums, provider led Boards and committees, Teach-Learn-Impact virtual learning series and on-site technical services, practices choose their level of involvement specific to their needs. Our goal is to assist with business practices and workflows to meet the demands of the payors while allowing clinicians the joy of working with their patients.

CARE COORDINATION
