HomeCare & Hospice of the Valley offers services that prevent and relieve suffering, restore dignity and comfort those living with a life-limiting disease. They base their care on compassion, comfort as they recognize and honor the dignity of each person. They serve patients and their families who reside in any of the local communities in the Roaring Fork, Colorado, Crystal, and Eagle River Valleys.
Their Home Health Services provides care for individuals with diseases such as Alzheimer's, Multiple Sclerosis, Parkinson's, cancer, heart disease, strokes, extensive and complicated wound care, as well as diabetes management. Through their Hospice services, they provide care to individuals who are diagnosed with a life-limiting illness who have a prognosis of six months or less to live. Lastly, their Private Pay Services offer private duty nursing by certified nursing aides or personal care assistants. Care provided is a 2-hour minimum to 24 hours/day, 7 days/week.
HomeCare & Hospice of the Valley is a non-profit organization, governed by a Board of Directors who live and work in communities throughout their service area. Start-up funding for HomeCare & Hospice of the Valley came from Aspen Valley Hospital, the Aspen Valley Medical Foundation, and Valley View Hospital who formed Hospice of the Valley in 2008. In December 2009, Vail Valley Medical Center requested that Hospice of the Valley assume ownership of the 20-year-old Mountain Hospice program.
The mission of HomeCare & Hospice of the Valley is to provide compassionate and dignified palliative and end of life care to patients and their families.
