“The CCA is an indispensable part of our managed care strategies to improve and demonstrate our healthcare value to our community, our employers, and payor partners. Through our participation in the CCA, we leverage our primary care services to manage population health, lower health insurance costs, and improve access to quality local healthcare. The CCA Clinically Integrated Network provides further opportunities for all member hospitals to work together, gain economies of scale, and enhance our quality of our healthcare services across Colorado. By so doing, we gain credibility and leverage with our payor partners to support the sustainability of our services and keep healthcare local.”

“The CCA has been instrumental in helping us start and grow our Primary Care practice into an advanced primary care practice. Right from the beginning, their support and resources were invaluable in setting up our operations and ensuring we had everything we needed to begin participating in the ACO. Their team has always been there to guide us through challenges and offer solutions that are both innovative and practical. Moreover, the ongoing support and educational opportunities provided by CCA have allowed us to continuously improve and expand our services. This has enabled us to stay ahead of the curve in an ever-evolving healthcare landscape. The collaboration and partnership with CCA have truly been a cornerstone in our journey towards excellence in patient care.”

“Staffing is a really big challenge in rural Colorado. Without CCA’s support, we would not have been able to keep our Chronic Care Management program going. With their support we not only maintained our enrollment in the program, but also grew the number of people receiving services, continuing to provide the added layer of care Medicare members need to help achieve improved health outcomes. CCA sustained us until we found the right team member to resume the work ourselves. We are tremendously grateful for their collaboration and partnership.”
Paula Belcher, MPH, M. Arch

“The CCA team is incredible to work with. They are professional, responsive, and knowledgeable not only on clinical supports to guide patients on managing their conditions, but also on implementing best practices to support our clinics in putting a care management program in place. From our first conversations to implementation, they have shown up as true partners, open to feedback and willing to dive in and problem-solve. They are also excellent at joining teams and partnering with providers remotely! We are excited for the many patients in our community who will improve their health outcomes because of this program.”
